Requests MUST be made within 10 days of the date of your arrest for Driving Under the Influence. Don't wait for the last minute. Be sure to keep a copy of fax transmittals and transmittal receipts or your letter requesting the hearing.
The DMV offices do not answer phones on weekends or holidays, so send a fax.
